Start beside the answers on your page.
Open Forms, then Design a form and choose + Single-choice question. Pagekind previews two circles. Place them beside answer labels already in the PDF. The designer adds fillable circles; it does not write visible question or answer text.
Name the question and its choices.
Give the question a unique name and a descriptive label / tooltip. Select an answer in the list and enter a different short name for each choice, such as Email or Phone. Choice names begin with a letter and can contain letters, numbers, underscores and hyphens. The name Off is reserved for an unselected answer.
Arrange the circles.
Drag an outline to move one circle or its corner to resize it. Escape cancels the drag. Exact circle position and size accepts page percentages and sizes from 8 to 72 points. Valid numeric edits preview immediately. Circles stay square and must fit inside the visible page, clear of other form fields. Add answer choice supports up to twenty choices. Move choice earlier or later changes its question-list order; it does not set PDF keyboard tab order.
Apply the question, then try it.
Choose a starting answer, or leave the question unselected. Answer required and Read-only answer are under Question rules. Add question to the page applies the complete group in one step. Try filling first applies a valid question draft, then opens the actual fillable fields. Selecting one circle clears the other selections in that question. Invalid drafts stay available to correct.
Make a change without losing the answer.
Select the question or an individual circle to change it. Answer in this copy is separate from Starting answer when the form is reset. Renaming or moving a choice preserves those selections. Before removing a selected or starting choice, select its replacement or No answer selected. Remove this answer choice and Remove entire question have separate actions. Apply question changes keeps the draft in one undoable step. Save a copy writes the finished PDF while keeping the original.
Know the current scope.
This designer supports its own complete single-page groups. Imported, scripted, grouped under another field, separately modified or tag-associated questions may remain available only for supported filling. It adds no PDF accessibility tags, custom tab order, submission service, scripts or certificate signatures. A required flag does not prevent saving an unanswered copy. Removing a question is not secure redaction.
Available in the unsigned Pagekind 0.65.0 engineering preview. Automated checks use unshown controls and saved PDF output on the development PC; human usability and clean Windows release checks remain pending.
